ENROLL YOUR STUDENT

If you are a Clark-Shawnee Local School District resident and are enrolling your Kindergarten through 12th Grade student for the first time at Clark-Shawnee, please follow the steps below.

Kindergarten registration for the 2024-2025 school year began June 3, 2024. 

Step 1: Register your student(s) in FinalForms. 

Step 2: Once you have completed the registration online, please bring the following documents to the Administrative Office to complete your student(s) registration:

1. Original Birth Certificate from the Health Department
2. Vaccination Records
3. Photo ID of parent/legal guardian
4. Proof of Residency (Gas, Electric, or water bill dated in the last 30 days, or current Lease/Mortgage papers)
5. Any custody paperwork or legal documents pertaining to the student(s)
6. At the Administrative office, you will complete a Records Request form for any new student entering grades 1st through 12th. 
7. The Free and Reduced lunch application is available through a link while completing FinalForms.

If you live outside of the Clark-Shawnee Local School District and you wish to Open Enroll your student for the first time at Clark-Shawnee, please follow the steps below.

2024-2025 Open Enrollment 

Clark-Shawnee Open Enrollment for students kindergarten through 12th grade for the 2024-2025 school year opened on April 1, 2024, at 7:00 am. If you would like your student to be considered for Open Enrollment in the 2024-2025 school year you must submit an application on or after April 1, 2024. You must submit one application per child.

You will receive a letter of Open Enrollment acceptance or denial by August 1, 2024. If you have questions, please call the Clark-Shawnee Administrative Office at 937.717.2401.
